Job Description
Location: KAGA
Job Purpose
The Caseworker is responsible for
providing direct support to vulnerable population through structured case management services
, ensuring their needs are assessed, addressed, and followed up in a safe, confidential, and ethical manner.
Key Responsibilities
Case Identification and Intake
- Identify and register eligible beneficiaries/cases.
- Conduct intake interviews and initial assessments.
- Obtain informed consent before proceeding with services.
Case Management
- Develop and implement individual case plans.
- Provide regular follow-up and support to beneficiaries.
- Ensure timely case documentation and updates.
- Close cases appropriately when objectives are met.
Referrals and Coordination
- Refer clients to relevant service providers (health, legal, psychosocial, livelihood support)
- Maintain and utilize referral pathways
- Follow up on referrals to ensure services are received
Protection and Safeguarding
- Ensure adherence to protection principles (confidentiality, do no harm, best interest)
- Maintain strict confidentiality of case information
- Identify and report protection concerns and risks
Documentation and Reporting
- Maintain accurate and up-to-date case files and records
- Use approved case management tools and databases
- Prepare weekly and monthly reports
Community Engagement
- Conduct awareness sessions on available services
- Support community outreach and mobilization
- Build trust with communities and stakeholders
Coordination and Teamwork
- Work closely with supervisors and program teams
- Participate in case conferences and review meetings
- Collaborate with partners and service providers
Qualifications and Experience
- Bachelor’s degree in social work, Psychology, Sociology, or related field
- Minimum of 2–3 years' experiencein case management or protection programming
- Experience working with vulnerable populations
Key Skills and Competencies
- Strong interpersonal and communication skills
- Ability to handle sensitive and confidential information
- Good documentation and reporting skills
- Empathy, patience, and problem-solving ability
- Ability to work under pressure and in field settings
